We are excited that you have decided to join us in a transformative journey and are taking the first step to begin therapy.
Just by making this important decision, change has already begun for you! We understand you may be feeling some anxiety about getting started. This is perfectly normal. Your clinician understands this process, and you will be provided a safe, private, and confidential environment to share with your clinician your personal goals for change.
As you might have already guessed, there is some paperwork that needs to be completed to begin this process. If you wish, you may download, print, and fill out the forms below and bring them to your first appointment. This can save a good amount of time. Or, if you'd rather, we can provide these forms to you to complete at your first appointment.
If there are any questions that you don't understand, or don't feel comfortable answering, no sweat! Just leave those questions blank and you can discuss your question or concern with your clinician.
